Obesity Toolkit—User Agreement

The Care Continuum Alliance has created the Obesity Toolkit for use by employers, health plans and other organizations interested in designing a benefit for obesity and comorbidities.

Completion of this user agreement is required to for access to and use of the Value-Based Benefit Design for Obesity and Comorbidities User Agreement, included in the Toolkit. Once the user agreement is complete, the download will be available.

The purpose of this user agreement is to allow the Care Continuum Alliance to quantify and qualify the audience for this toolkit and to ensure that updates or errata, should any become available, reach users. The Care Continuum Alliance respects your privacy and will not share this information with third parties.
